What transition planning means
Information in easy read. When you should start thinking about your transition plan and what support you'll get.
About transitioning
Transition means moving from childhood to becoming a young adult.
Transition planning normally starts around the age of 14.
We call this transition planning.
It explains what everyone will do to make sure you get the support you need as you grow up from a teenager into an adult.
It doesn’t replace an EHC plan.
How the plan is put together
You, your parents, carers and other people who support you will go to a meeting at your school in year 9.
At the meeting you will all talk about:
- what you and your parents think needs to happen
- your education, health and social care needs
- the support you need as you grow up into an adult
- the support your parents, carers and family need
- if you need a Child’s Needs Assessment
What happens next
After the meeting, your school will put together the Transition Plan and send it out to everyone involved.
The plan will be updated every year until you leave school.
If we think you may need adult social care support when you’re over 18 years old, we'll start planning for that when you’re 16 years old.
Life transitions
There are transitions throughout your life that you may need help with.
They include:
- leaving college
- moving out of your family home
- getting your first job
- having your first relationship
- getting older
- moving into other housing and support – for example into residential care
Who can help you with these transitions
