When you should start think about your transition plan and what support you'll get.
Transition means moving from childhood to becoming a young adult.
Transition planning normally starts around the age of 14.
Your social worker will be able to help you think about what you want and what options you have.
Your college may also be able to help you think about becoming more independent.
