You do not need to fill in the form but it helps us to know what teams you'd like to receive information from. 

You'll need to provide a copy of your identification (ID). We need your ID so we can make sure that we don't release your personal information to anyone else. The types of identification we accept are listed on the form.

If you do not want to fill in the form send an email to the Information Governance Team and include:

• your full name
• your ID
• what information you would like - the more detail the better
• which teams within the council you would like this information from
