Community Navigation

Supported by Impetus, local community navigators work in GP surgeries to assess patients' non-medical support needs and help them access groups, services and activities that can broadly improve their health and wellbeing. The Navigator offers up to 6 appointments of around 45 minutes each, which mostly take place in the GP surgery, or in the person's home if they are housebound. Navigators are volunteers with a background in helping people meet their social or support needs.
