This form is for stonemasons and their agents only.

Complete this form if you want to place a new or replacement memorial on a grave. 

This must be signed by the grave owner. If the grave owner is deceased, you must apply for a transfer of ownership first.

You need to include:

• an inscription
• a description or drawing, including measurements, of the proposed memorial
