How to use this action sheet

Use this action sheet to sign up to meet the council leader in person and share your ideas, raise concerns or pose questions directly.

You must download the action sheet onto your computer first and save your own copy, then you'll be able to edit it and add your information.

We'll need to know:

• your name
• your address
• your email address
• details of the main issue being raised (for example, Housing, Benefits, Debt, Parking) – please be as specific as possible
• the outcome you are hoping for

You must also complete the consent/data protection notice on page 3.
