Carers contribution to a Me and My World review for a looked after child
If you are a carer, use this form to contribute information to your child's Me and My World review and plan.
About this form
You'll need to tell us:
- the child's name
- your full name and email address
- the dates of the child's last medical, optician's appointment and immunisations, and if there were any issues
- details of how you feel the child's experience has been overall over the last 6 months
- the dates of when you last:
- spoke to the child's school or nursery
- attended a parents' evening
- received a report from school
- attended a PEP meeting or educational review
