What you can request
This form allows you to:
- tell us about a temporary or permanent change of vehicle
- end a temporary change of vehicle
- tell us about an issue with the start or expiry date of your permit
- ask us to replace a lost, damaged or stolen permit
- ask us to cancel your permit
What you'll need to apply
All change requests
For all change requests you'll need your:
- permit number
- vehicle registration mark
Permanent change of vehicle or cancel your permit
To tell us about a permanent change of vehicle or to cancel your permit, you will also need to upload a photo of your permit cut up into 4 pieces. Without this, your request will be delayed or rejected.
Temporary change of vehicle
To tell us about a temporary change of vehicle, you will also need the temporary vehicle registration mark.
Start or expiry date issue
To tell us about a start or expiry date issue, you will also need to tell us the reason you're requesting a change.
